The Alzheimer's Art Quilt Initiative is asking for your help in creating an exhibit of textile art called "Alzheimer's Illustrated." We are creating 182 long and narrow "Name Quilts" which will be the "backdrop" for 52 small quilts about Alzheimer's. Each Name Quilt will have 55 patches with a name of a person who has/had this disease (or a related dementia) in each patch. The 10,000 names will represent the 5.3 million Americans with Alzheimer's disease.

We are asking for your help in collecting names.


When you "rent" a Name Quilt top, you are agreeing to do the following:

  • Collect 55 names of people who have/had Alzheimer's, written by their family or friends.
    Take the quilt top anywhere you like to gather the names, however you must be present when people write the names of their loved ones.
  • Show the Letter of Introduction explaining the project when you ask people if they would like to participate. Click here to view/print it.

  • Have people write ONE name per patch, on the WRONG side of the fabric, with PERMANENT marker. ALL NAMES MUST BE FACING IN THE SAME DIRECTION.
    You must provide the markers.
    A thick line is best as names will be read from a distance.
    Black ink is preferred but you may use other colors so that the name can be seen on the dark patches.
    Bring something to put UNDER the cloth as the ink will bleed through.
    Do not write within 1/2" of the raw edges.
    Include city/state where the remembered person is from.
    Do not skip patches.
    If you run out of patches have people write the names on paper and mail paper back with quilt top, or rent another quilt top.
  • Mail the quilt top in the priority envelope provided at your expense within 30 days of receiving it.


Please note: Quilt Tops are sent out priority mail and a shipping charge of $4.95 will be added to your order when you "check out" using the shopping cart on this web page. Although it usually only takes two to three days, it could take up to 10 days to reach you. Volunteers normally process orders within a few days of receiving them. Please plan accordingly.
